Required materials:

1. The person placing out the child for adoption submits the materials and files the application

(1) Photos of the adoptee (including 4 pieces of 2-inch ID photos and 2 pieces of 5-inch life photos); 

(2) the adoptee’s household registration certificate (1 photocopy); 

(3) the adoptee’s certificate of physical examination( 1 original); 

(4) the household registration certificate and resident ID card of the adopter (1 copy for each document); 

(5) the certificate of family relationship between the person placing out the child for adoption and the adoptee issued by the public security organ or notary office (1 original); 

(6) the certificate of family relationship between the person placing out the child for adoption and the adopter issued by the public security organ or notary office (1 original, requiring the type of family relationship and the related person to be clearly indicated); 

(7) the certificate stating that the adoptee is the child of a collateral relative by blood up to the third degree of kinship of the adopter issued by the public security organ or notary office (1 original); 

(8) the adopter’s declaration and notarization agreeing to the adoption (1 original);

(9) if the adopter is a spouse, a marriage certificate (1 photocopy) shall be issued; 

(10) if the adoptee is over 8 years old, a declaration and a notarization of being adopted on a voluntary basis shall be issued (1 original)

2. The foreigner entrusts an adoption organization to submit the materials and file the application

(1) Notification of adopting children in China issued by the Chinese adoption organization (2 copies);

(2) the adopter’s ID card (2 originals and 2 photocopies);

(3) the adopter’s photo (2 inches, 5 pieces); 

(4) notification stating that the adoptee has been approved for adoption (2 copies); 

(5) resident household register and resident ID card of the person placing out the child for adoption; 

(6) photo of the adoptee (2 inches, 5 inches, 6 copies for each size)
